Medical Associates of North Georgia - Riverstone DOT Physical

This option is for Cherokee County School District bus drivers who need to complete their required annual DOT Physical.

Please bring a photo ID, any required DOT forms, and a list of your current medications to your appointment. If you wear glasses, contacts, or hearing aids, please bring those as well.

Patients that are currently being treated for diabetes, cardiovascular, pain management, or insulin dependent diagnoses will need to have their treating provider fill out the appropriate form(s) prior to your DOT appointment with Medical Associates.
